Cancellation Policy
Neurotoxin Treatment Policy for Sleek IV & Wellness To ensure the best experience for all clients, please review our neurotoxin treatment policies before booking. Booking & Deposit Policy A $100 non-refundable deposit is required to reserve your appointment. The deposit will be applied toward your treatment cost on the day of service. Cancellation & Rescheduling Appointments may be rescheduled one time with at least 24 hours’ notice. Cancellations, no-shows, or rescheduling requests made less than 24 hours before the appointment will result in forfeiture of the deposit. Consultation & Treatment Eligibility All neurotoxin treatments require a medical assessment by our Board-Certified Family Nurse Practitioner to determine eligibility and create a personalized treatment plan. Treatment recommendations may vary based on anatomy, muscle strength, medical history, and desired results. Pricing Pricing listed online reflects starting treatment plans. Final treatment costs are based on the treatment area(s), product selected, and number of units required to achieve optimal results. Additional units may be recommended during your consultation and will be discussed before treatment. Results & Follow-Up Results typically begin to appear within several days and continue to develop over approximately two weeks. Individual results may vary. A follow-up evaluation may be recommended to assess treatment outcomes. Post-Treatment Evaluation Touch-up treatments are not guaranteed and may incur an additional charge if more product is required. Any concerns regarding treatment results should be communicated to the clinic within 14 days of treatment. Late Arrivals Clients arriving more than 15 minutes late may need to be rescheduled, and the deposit may be forfeited. Refund Policy Due to the nature of neurotoxin treatments and the inability to return or reuse medication once prepared and administered, all neurotoxin treatment sales are final. No refunds will be issued for services rendered. Pregnancy & Breastfeeding Neurotoxin treatments are not performed on clients who are pregnant, planning pregnancy, or breastfeeding. Medical Disclosure Clients are responsible for providing complete and accurate medical information, including medications, allergies, previous neurotoxin treatments, and medical conditions that may affect treatment eligibility. By booking an appointment, you acknowledge that you have read and agree to these policies.
