Skip the insurance runaround. Sleek IV & Wellness offers transparent, cash-pay lab work in Prosper for anyone who wants answers without a referral, a deductible, or a surprise bill. What's included: In-clinic blood draw by a nurse practitioner with 9+ years of critical care experience Processing through a CLIA-certified reference lab (CPL) Results reviewed by your provider with clear next steps - not just a PDF HSA/FSA accepted Popular panels: Iron workup (CBC, CMP, ferritin, full iron panel) Hormone panels (testosterone, estradiol, progesterone, thyroid) Weight loss baseline labs Wellness & metabolic screening (lipids, A1c, vitamin D, B12) How it works: book a 20-minute draw, arrive fasting if your panel requires it, and results are typically back within 2–4 business days (hormones usually take about a week). Cancellation Policy
Cash-Pay Lab & Blood Work - Booking Policy 1. Payment: Full payment is due at booking. All lab pricing is cash-pay and includes the blood draw, lab processing, and provider review of results. We do not bill insurance. HSA/FSA cards are accepted. 2. Insurance draw option: If you select "Draw Fee Only," you pay only the $35 draw fee to Sleek IV & Wellness. Your lab work is sent to our reference lab, which bills your insurance directly. Bring your insurance card and any outside lab order to your appointment. Any deductible, copay, or non-covered charges are billed to you by the lab, not by us, and we cannot guarantee what your plan will cover. This option is not available for Medicare or Medicaid. 3. Cancellations and rescheduling: Please give at least 24 hours' notice to cancel or reschedule for a full refund or credit. Cancellations with less than 24 hours' notice and no-shows forfeit $20 of the amount paid; the remainder is refunded or held as credit. You may reschedule once at no charge with 24 hours' notice. 4. Fasting: Some panels require 8–12 hours of fasting (water is fine). We'll tell you when you book. If you arrive non-fasting for a fasting panel, we can draw non-fasting with your consent or reschedule without penalty. 5. Draw attempts: If we're unable to obtain a sample after two attempts, you may reschedule at no charge or receive a full refund. 6. Results: Most results return in 2–4 business days; hormone panels may take up to a week. Your provider will contact you to review any abnormal results and recommend next steps. Results are released through CPL patient portal. Results are not a substitute for an emergency evaluation - if you feel acutely unwell, seek care immediately. 7. Custom labs: Custom (priced at visit) covers the draw fee only. Additional panels ordered by your provider are quoted before collection and charged at the visit. 8. Refunds after collection. Once your sample has been collected and sent to the lab, the service is non-refundable. **Age.** Patients under 18 require a parent or guardian present.
