Service Description
Our Follow-Up Visit & Medication Refill service is designed for established patients who need continued care, treatment adjustments, or prescription renewals. This visit allows your provider to review your response to treatment, assess symptoms, monitor progress, and determine whether medication refills or dosage changes are clinically appropriate. Visits are conducted via secure telehealth, making it easy to stay on track without an in-office appointment. When indicated, medication refills may be provided, and treatment plans can be updated to support your ongoing health goals. All follow-ups are completed by a board-certified Family Nurse Practitioner, ensuring safe, evidence-based, and personalized care.
Cancellation Policy
At Sleek IV & Wellness, we respect your time and appreciate your cooperation in helping us provide quality, convenient care to all clients. Appointments: Visits can be booked online, by phone, or via text. A $50 deposit is required to secure your appointment and will be applied toward your total visit cost. You will receive a secure link to complete the intake paperwork and for your virtual consultation once your appointment is confirmed. Cancellations & Rescheduling: We require at least 24 hours’ notice for cancellations or reschedules. Appointments canceled with less than 24 hours’ notice will forfeit their $50 deposit. No-shows will be charged the full appointment fee.
