As a board-certified Family Nurse Practitioner, Yvonne takes a full history, reviews your symptoms and goals, and orders labs to guide your plan - not guesswork. This evaluation is right for you if you're interested in hormone replacement therapy (HRT), managing a chronic condition, or general wellness optimization. Looking for medical weight loss? Cancellation Policy
At Sleek IV & Wellness, we respect your time and appreciate your cooperation in helping us provide quality, convenient care to all clients. Appointments: Visits can be booked online, by phone, or via text. A deposit is required to secure your appointment and will be applied toward your total visit cost. You will receive a secure link to complete the intake paperwork and for your virtual consultation once your appointment is confirmed. Cancellations & Rescheduling: We require at least 24 hours’ notice for cancellations or reschedules. Appointments canceled with less than 24 hours’ notice will forfeit their deposit. No shows will be charged the full appointment fee.
